The Document Center lets you collect signed forms and identity documents from a client without them coming into the office. From a client’s Documents tab, you send a secure link; the client opens it on their own phone, completes the form or takes a photo, and the result is attached back to their record. Open a client profile, go to the Documents tab, and use the Document Center to send a request. Each request creates a single-use, expiring link tied to that client.

Forms a client can e-sign

The Document Center can request any of these signable forms. The client reviews the form, confirms their details, types their name, and signs on-screen:

Marketplace Release Acknowledgement

Enrollment & Coverage Acknowledgement

Income & Eligibility Confirmation

DCF Access Authorization

FFM Consent

How the client completes a form

1

Open the link

The client taps the link you sent. The page opens with no login — the link itself identifies them — and shows the form with their on-file contact and case details for review.
2

Review and sign

The client confirms the information, types their full name, and signs in the signature box.
3

Submit

On submit, the signed form is recorded against the client’s case and the link is consumed.
Each link is single-use and expires. If a client opens a link that has already been submitted, expired, or is invalid, they see a message asking them to contact your office for a new one. Every completion is logged for the audit trail.

Capturing a government ID

The Document Center can also request a photo of the client’s government-issued ID. The link opens the client’s camera with an on-screen frame sized to the document:
1

Open the ID link

The client taps the link. Their device camera opens (rear-facing) with a guide frame.
2

Line up and capture

The frame is shaped for the document type — an ID card / driver’s license or a passport page — and can request a specific side (front or back). The client fills the frame and captures.
3

Review and upload

The client checks the preview and uploads. If the camera is unavailable, they can pick a photo from their device instead.
The captured image is attached to the client’s documents automatically — no legal signature is involved, since this is an upload rather than a signable form.
Both form links and ID-capture links are the same kind of secure, client-held link used by the kiosk and are safe to send by text or email. They never expose the client’s record — only the specific form or upload you requested.
